At Actualize Health we utilize a number of different modalities to engage patients and get them scheduled for wellness visits, preventive services, and other important health-related services.
Quality Improvement
Demonstrating the high-quality care provided to patients is a key component in value-based models of care. Actualize can help practices capture and communicate quality data. Additionally, our model focuses heavily on reaching out to patients and scheduling needed services so overburdened and understaffed practices don’t have to.
Value-Based Incentives
There are an increasing number of financial rewards made available for achieving quality targets and reducing avoidable health care expenditures. Actualize Health contracts with major health plans to make these financial arrangements available to practices and then provides the support necessary to achieve agreed-upon targets. At Actualize Health, we believe these financial incentives are critical to augmenting fee-for-service revenue and making it financially viable for practices to care for underserved patients.
Social Determinants of Health
Social factors like housing, access to healthy food, and transportation play a major role in patient compliance and health outcomes. Identifying these factors is the first step to addressing them. Using validated survey tools, Actualize Health can routinely screen for and document patients’ social needs. Utilizing this data, Actualize identifies and partners with local community-based organizations and can provide staff to navigate patients to needed resources.
