Why
Actualize Health
- Value-based arrangements have focused heavily on Medicare and Commercial patient populations. Underserved patients and their physicians deserve the same level of attention and support to improve outcomes.
- Fee-for-service payment from Medicaid and other government programs is not enough to sustain the safety net in our communities. Practices caring for underserved patients need value-based payment models to remain viable and maintain critical access to care.

More Than Reports
- Performing well in value-based arrangements can take more time and effort from practices. Many value-based organizations are happy to give you reports and highlight all the work you and your practice need to do to perform better. When you work with Actualize Health, we use our staff to outreach to patients, schedule appointments, and follow-up on your behalf. We work closely with you and your staff, but we refuse to dump more work on your plate.
- Value-based care and fee-for-service are not mutually exclusive. Value-based contracts with insurance companies can offer some very large financial rewards for practices. That said, those rewards often take 12 to 18 months to be paid out. In the interim, Actualize Health’s patient outreach drives more visits back into your practice, generating critical billable encounters. Additionally, Actualize Health staff generates practice revenue throughout the year via a number of payer-sponsored incentive programs. We believe it takes visits and volume throughout the year to support patients and practices.

Dr. Jim Walton is President of JWalton, LLC a healthcare consultancy firm with expertise in physician-owned value networks, risk-bearing accountable care organizations, and safety net care delivery focused on sustainability in patient-centered healthcare financing, delivery redesign, and health equity improvement.
Jeff Zsohar, MD
Dr. Jeff Zsohar is the Medical Director of Baylor Scott and White Community Care, a network of seven clinics caring for uninsured and low-income patients in North Texas. The Community Care clinics use integrated care teams to address medical, behavioral, and social determinant of health risks for underserved patient populations.
